Our advocacy program offers enrollment assistance, information, and education to those interested in receiving the services under the Home and Community Based Waiver.

The process for enrollment can be long and confusing, we can assist with paperwork, liaison with your doctors, remind you of appointments, and ensure the process isn’t too confusing or overwhelming. We want you to focus on what is important, and that is your health.

    Once approved for services, the Community Health Choice – MCO will coordinate your health care coverage to improve the quality of your health care experience — serving more people in communities rather than in facilities, giving them the opportunity to work, spend more time with their families, and experience an overall better quality of life. You will be provided with three choices; Keystone First CHC, PA Health & Wellness, and UPMC.
